State Health Plan
Solution to Help Transform Tennessee’s Long-Term Care
Program
CINCINNATI, Ohio - Oct. 15, 2009 – CH

Health Plan (VSHP) has signed a contract to implement CH
Mack’s fully integrated care management software suite, Q
Continuum System™, to enhance care coordination for
Tennessee’s Long-Term Care (LTC) population.
VSHP, a wholly-owned subsidiary of BlueCross® BlueShield®
of Tennessee, is one of the largest TennCare MCOs serving
nearly 500,000 members. CH Mack's integrated care management
solutions are used by more than 75 organizations serving
over 8 million covered lives nationwide.
CH Mack's Q Continuum System will be used by VSHP to
manage the provision of quality health care services for
enrollees in TennCare’s Long-Term Care Community Choices
program. TennCare is Tennessee’s Medicaid managed care
program. It is the only program in the nation to enroll the
entire state Medicaid population in managed care. In 2008
the passage of landmark legislation - the Long Term Care
Community Choices Act - paved the way for a record number of
Tennesseans, who would otherwise need nursing home care, to
receive Home and Community Based Services (HCBS) and remain
independent in their homes.
