Q Conference Recap

In spite of adverse weather conditions, the Q Continuum National Conference was January 16, 17 and 18, 2007 in Austin, TX, hosted by The Area Agency on Aging of the Capital Area.

Agenda topics included; Information Technology as a Tool for Emeregency Response for the Elderly...a Rita Case History, presented by Glenda Rogers of The Area Agency on Aging of the Capital Area; Enhanced Information & Referral Solutions for Improved Care Coordination by David Greene of North Light, Inc., Q Tips and Techniques; Planned Enhancements and Future Plans & Directions of Q.

Catherine Mullahy, President/Owner of Mullahy Associates, LLC, our Keynote Speaker, presented a follow-up Webinar shortly after the conference for those who were registered to attend but were unable to make it.

The conference was a success - and thank you to all who contributed. An extra special THANK YOU goes out to Glenda Rogers, Ray Glenn and the rest of Glenda's team at The Area Agency on Aging of the Capital Area. We couldn't have done it without you.

  • Customer Spotlight - Pinal/Gila Long Term Care
  • Pinal/Gila Long Term Care (P/GLTC), a Medicaid waiver managed care system, is dedicated to the provision of innovative, culturally sensitive, community focused programs specializing in consumer driven health care and support services for the frail elderly, individuals with physical disabilities and/or chronic health conditions. P/GLTC strives to enhance the quality of life of its members by promoting individual choice, accessible services, dignity and independence while reducing the costly effects of illness on our members and the communities it serves.

    P/GLTC provides services to members in Pinal and Gila Counties in Central Arizona. In addition to covering all medical care, services encompass: Adult Day Health Care, Attendant Care Services, Family Attendant Care Services, Emergency Alert System, Group Respite Care, Habilitation, Home Modification, Home Delivered Meals, Home Health Service, Homemaker, Hospice Care, Personal Care and Respite.

    Various types of living arrangements are available for P/GLTC members who need them: Adult Foster Care, Assisted Living Homes, Assisted Living Centers, Behavioral Health Group Homes, Skilled Nursing Facilities, and Traumatic Brain Injury Treatment Group Homes.

    P/GLTC selected Q Continuum to support its case management process, going live in 2004, and has been a loyal customer since.

    The Check-In/Check-Out feature of Q is quite beneficial to P/GLTC. As the vast majority of the population for these counties is rural, this feature is very handy to the case managers by allowing them to have direct access to a member's data while in the field.

    The Disease Management feature in Q allows P/GLTC to manage their Special Health Programs. This feature presents a customer configurable assessment (as determined by their administrator) for specific diseases. These questions are geared towards assessing the client's current condition, evaluating trends and determining a course of action.

    Q Continuum has provided all of the functionality required to support the clients P/GLTC serves.   • This Month’s Q-Tip…..Search History
  • Q Continuum System

    Changes, changes and more changes. Keeping up with the current information flow can be a timely and frustrating task. Your life just got easier. Q Continuum now offers a Search History Function.

    Say you are going to search for Mr. Smith's phone number, 123-456-7890. Nothing comes up because Mr. Smith's new phone number is 098-765-4321. You can click on the Search History checkbox, which will give you all possible matches. You can then bring up the current information.

    The Search History function works in the screen that you are currently in. So if you are in 'Client', that is the historical data it will search. If you're in 'caregiver', it will search historical data for caregiver, and so on.

  • CH Mack Presents Technology Leadership Award To Clermont Senior Services
  • Cincinnati, Ohio – April 3, 2007 – CH Mack, Inc., presented our Technology Leadership Award to Clermont Senior Services at their Annual Meeting on April 3, 2007, in Batavia, Ohio.

    Mr. Edward Carl, President/CEO of CH Mack, Inc., recognized Clermont Senior Services for effective use of technology in providing service to seniors. The award was received on behalf of Clermont Senior Services by Mr. George Brown, Executive Director; Mr. Greg Shivener; Director of IT; and Mr. Jeff Fee, Director of Business and Finance.
