Q National Conference

The Q Continuum National Conference is scheduled for January 16, 17 and 18, 2007 in Austin, TX. The Keynote Speaker is Catherine M. Mullahy, RN, BS, CRRN, CCM, President of Mullahy and Associates, LLC.

Catherine’s more than 35 years of experience in the health care profession has made her a nationally recognized case management consultant and a spokesperson for the case management industry. A nationwide speaker/trainer, she also serves as editor and contributing editor to major case management publications and has authored an authoritative handbook on case management (Titled “The Case Manager’s Handbook), and numerous articles on health care.

We will facilitate sessions to encourage peer to peer exchange and provide an environment where you can share innovative ideas and learn best practices. The meeting agenda includes discussions on topics such as: Best Practices, Tips & Techniques, Planned Enhancements, Future Plans and Directions for Q.

The Area Agency on Aging of the Capital Area, a program of the Capital Area Council of Governments, is housed in a new facility in the Live Music Capital of the World, Austin, Texas, and has graciously offered to host our National Q User’s Group.     The Area Agency on Aging of the Capital Area, a program of the Capital Area Council of Governments, located in Austin, TX, went live with Q Continuum in September 2003. The Area Agency on Aging of the Capital Area (AAACAP), whose mission is to promote independence for people sixty years or older and their caregivers living in Austin and the surrounding the-county Capital Area. Through the Area Plan for Aging Services, AAACAP assesses the needs of the region’s older adults and develops care strategies to meet them. Funded by the Texas Department of Aging and Disability Services (DADS), services are provided without cost to eligible residents of Bastrop, Blanco, Burnet, Caldwell, Fayette, Hays, Lee, Llano, Travis and Williamson Counties.

    CH Mack was contracted to implement Q Continuum System to support the care management and service delivery processes of several Texas Area Agencies on Aging (AAAs). Many Texas AAA’s are utilizing Q Continuum System to support the following programs: Caregiver Support, Care Coordination Support for Hospital Stay Recovery, Information and Referral and Assistance, Benefits Counseling, Health Maintenance, Nutrition and Transportation. Q Continuum System is being deployed to Texas AAA care managers via the Web, with the solution hosted by an Application Service Provider (ASP) that provides all IT infrastructures, software maintenance and support services. From a user perspective, Q Continuum System access only requires an Internet browser. Deploying Q Continuum System, using an ASP, has allowed the Texas AAAs to implement the system rapidly and at significantly reduced upfront costs.

    For the Capital Area Council of Governments, Q Continuum System training was provided and the system was operational in less than one month. Since the original “go-live”, CH Mack has enhanced Q Continuum System for the consortium of Texas AAAs, including the creation of Texas government specific forms (e.g. the Texas DHS-2060 for tracking ADLs/IADLs) and reports (monthly service reports and quarterly reports) in the Texas specific formats. As a result of the successes, attributed to the use of Q Continuum System by the Area Agencies on Aging, the utilization of Q Continuum System is being expanded to cover other human services departments within their regional Council of Governments (COGs). processes in their organization." said Ed Carl, CEO, CH Mack, Inc.

    Data integration processes have been created for the Texas agencies to automate the export of information to both State and Federal systems, to fulfill government reporting requirements. Currently Q Continuum System is being used to support 160 care management professionals across 13 government agencies to support the needs of 100,000 older adults in Texas.

    Q Continuum System has proven to be an invaluable tool in the support and relief efforts implemented in Texas to provide assistance to the victims of Hurricanes Katrina and Rita. The system has been utilized by Emergency Response personnel to support Disaster Preparedness and follow-on care management efforts, in response to the hurricanes. During this critical period of time, the laptop Check- in/Check-out version of Q Continuum System has allowed care professionals to work in remote regions of the state, with full access to the care management functions provided by the system. This has expedited the delivery of services to those most in need.

  • Q Continuum System

    Are you tired of creating the same document for multiple clients, over and over? Start taking advantage of a feature built into Q Continuum that makes performing administrative tasks less tedious.

    Q Continuum supports a mail merge function that provides the ability to create a template in MS Word and then generate client specific documents from records in Q. This allows the user flexibility to create client communication, direct mail, address labels, welcome letters, etc.

    Information can be segmented to produce documents for one client or a group of clients. For example, you have a provider that is going to host a nutritional seminar but not all of your clients need to attend this seminar. You can segment your clients with high nutritional risk scores to send them an invitation. Q mail merge can be used to generate the invitation letters and mailing labels.
