You're Invited To The Future: Q National User Conference - October 2, 3, 4 in Cincinnati, Ohio

The CH Mack Management Team invites you to the October 2007 Q National User Conference. The goal of this conference is to provide Q Users - at all levels - with an environment of sharing innovative ideas, learn best practices, and have a peer-to-peer exchange. Not only will you get a sneak peek at Planned Enhancements and the Future Plans and Directions for Q, you will also have an opportunity to provide input for those future plans and directions.

Ms. Ellen Aliberti, Director of Post Acute Services for Sierra Health Services, Inc., has a wealth of industry experience and knowledge. Her presentation, Going from Q-less to Quru (Clueless to Guru), will kick the conference off to a great start.

You are free to bring other team members in addition to yourself (or if you are unable to attend, in place of yourself). However, seating is limited - so please register now. ·  Customer Spotlight - WISE Senior Services

 

WISE Senior Services is a well-established, multi-service, private, non-profit organization serving senior citizens throughout Los Angeles County, with emphasis on low-income and/or underserved individuals. Their mission is to promote and improve the well-being, independence and self-esteem of seniors, and to prevent inappropriate institutionalization whenever possible.

Initially, WISE stood for Westside Independent Services for the Elderly, but over time, as a reflection of their mission, it has come to stand for Well-being, Independence, Self-esteem and Education.

WISE was founded in 1968 by volunteers and community members to address the needs of the growing senior population in Los Angeles County. During the first few years of operation, WISE was instrumental in establishing the first senior center in Santa Monica in 1972. In the 1980's, WISE began its involvement with the Long Term Care Ombudsman Program, opening the first three of the 11 offices this program currently has throughout the County. Since then, WISE has expanded its scope of services to reach a variety of seniors in need: those who are at risk when alone, have a physical or mental impairment, want to remain in their own home, are lonely or depressed and/or are at risk for exploitation and abuse by others.

Over the years, WISE has developed several model programs for seniors and their families, most notably related to elder abuse issues, which have gained national attention and recognition. In 2002, after many years of researching best practices, WISE opened a new state-of-the-art building for their adult day care program that was designed specifically to meet the needs of the frail elderly and that nearly doubled their service capacity. Also, in 2002, WISE was named "Agency of the Year" by the City of Los Angeles in recognition of the innovation and creativity of its programs. WISE has a long history of involvement in associations and task forces and frequently shares its expertise in policy development.

WISE Senior Services went live with Q Continuum Case Management in October of 2005; the Transportation Module was added in 2006. From the first day a call comes in, WISE can track how many intakes are done in a month. Cases are emailed, reviewed and assigned; funding sources can be tracked by category - all via Q Continuum System. Even volunteers are trained on how to use Q.

Q has enabled WISE to gain better control of their services and produce better outcomes for their patients and bottom line. Q Continuum supports the Case Management Department from the point of referral through program discharge. The case managers are able to easily complete the required state assessment forms and communicate the information with ease. Q's reporting abilities have allowed WISE to manage and track daily activities such as referrals, case manager assignments and funding sources.
